This is a fully remote position, open to applicants in New York.
• Assist in retaining patients and maintaining effective communication regarding ongoing patient services.
• Place medication orders and manage chronic disease-state medications for home delivery.
• Act as a liaison between Regional Care Coordinators, patients, physician offices, referral sources, Intake, Pharmacy, and Reimbursement teams.
• Monitor patient services that impact the quality, safety of care, and patient retention.
• Collaborate with reimbursement and pharmacy departments to ensure accurate medication orders, prompt delivery, care, and follow-up.
• Utilize phone, computer, and other communication tools, including aiding during patient emergencies when necessary.
• Keep precise and timely patient records and notes in CPR+.
• Receive and input patient referrals efficiently.
• Attend consumer group events and support the sales team during physician visits or local/regional events as needed.
• Participate in biweekly specialty sales update calls.
• Maintain regular monthly contact with patients.
• Generate new and refill patient orders as required.
• Provide delivery services when necessary and approved.
• Assist in searching for alternative reimbursement resources.
• Collect updated patient information, such as statistics, allergies, health issues, and insurance changes.
• Track and confirm deliveries, obtaining signatures and documenting the process.
• Address delivery issues and assist with medication recalls.
• Enhance processes and service quality.
• Maintain regular communication with the direct supervisor.
• Adhere to company policies, safety regulations, and HIPAA compliance standards.
• High School diploma or GED is mandatory.
• At least two (2) years of experience in a high-volume call center setting is required.
• Proficient in computer and internet skills, particularly with Microsoft Office Suite.
• A college degree is preferred, but not essential.
• Bilingual abilities are advantageous but not required.
• Experience in health promotion/maintenance is preferred but not mandatory.
• Strong understanding of hemophilia, immune deficiency, IG-treated chronic disease states, and similar chronic conditions.
• Knowledge of IG and hemophilia treatments along with infusion administration techniques.
• 3–5 years of experience in the chronic disease/disorder community is preferred.
• Excellent customer service, interpersonal, organizational, verbal, and written communication skills.
• Proficient in the necessary programs to perform the job effectively.
• Capability to adhere to legal, ethical, compliance, HIPAA, and company policies.
• Ability to occasionally stand, walk, sit, climb stairs, balance, stoop, kneel, crouch, crawl, and lift or move up to 25 pounds.
• Capacity to thrive in a fast-paced environment, prioritize competing demands, manage multiple tasks, and seek guidance when necessary.
