Patient Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Greet and assist patients, customers, and vendors.

• Act as the primary contact for patient inquiries, requests, and general questions.

• Handle incoming calls, emails, and digital communications.

• Schedule, confirm, and reschedule patient appointments as needed.

• Optimize provider schedules and help fill same-day openings.

• Assist with appointment reminder and confirmation processes.

• Verify patient demographics and insurance eligibility while maintaining accurate EMR records.

• Provide assistance with questions regarding insurance coverage, benefits, and financial responsibilities.

• Prepare patient charts and necessary documentation.

• Process co-pays, payments, charges, and account information within the EMR system.

• Monitor patient flow and provider schedules, addressing any delays, gaps, and scheduling conflicts.

• Keep patients updated on wait times, delays, and next steps in their care.

• Coordinate with clinical and optical teams to ensure smooth patient transitions and continuity of care.

• Enhance patient retention through follow-up, recall, and reactivation outreach efforts.

• Maintain a clean, organized, and patient-ready front office environment.

• Carry out additional duties as assigned by the Practice Manager, Assistant Practice Manager, and/or Optometrist.
