Patient Care Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Manage monthly refill requests from patients and ensure all documentation is precise before dispatching orders.

• Process orders for new patients and maintain updates in electronic medical records.

• Collaborate with the patient admissions team to facilitate the onboarding of patients to services.

• Respond to incoming inquiries from patients, caregivers, branches, and referral sources concerning the status of nutrition therapy orders.

• Handle modifications to prescriptions.

• Direct calls to the appropriate departments as needed.

• Confirm patient demographics and insurance details to guarantee accurate order shipment.

• Initiate the referral process and address questions from patients and caregivers regarding the status of admissions.

⛳️ Requirements

• At least one year of experience in a customer service role.

• Proficient in data entry.

• Familiarity with Microsoft Office, particularly Excel, Outlook, and Word.

• High School diploma or GED is required.

• Availability to work Monday through Friday, from 10:00 AM to 6:30 PM Central Time.

• Preferred: Experience in home infusion or durable medical equipment (DME).

• Preferred: Call center experience.

• Preferred: Background in a healthcare setting.
