This is a fully remote position, open to applicants in Kentucky, +1 more state.
β’ Serve as the advocate for patients to ensure that prescribed medication therapies and pharmacy care plans are adhered to.
β’ Provide courteous, professional, and high-quality service.
β’ Collaborate with healthcare providers, payers, and other health-related resources to coordinate pharmacy care.
β’ Address and resolve patient inquiries within established time frames.
β’ Assist patients with issues related to drug therapy and pharmacy care.
β’ Encourage adherence to prescribed medication regimens.
β’ Report any adverse events linked to drug therapy and pharmacy care.
β’ Foster and maintain relationships with physician practices while enhancing communication between patients, payers, pharmacists, nurses, laboratories, and other healthcare professionals.
β’ Engage in the administration of patients' pharmacy care plans.
β’ Identify and resolve issues faced by patients and payers.
β’ Support fellow healthcare team members in a collaborative environment.
β’ Recognize opportunities for quality improvement and convey recommendations.
β’ Keep accurate notes and documentation concerning patients' drug therapy and pharmacy care plans.
β’ Coordinate responses and solutions with relevant internal and external stakeholders.
β’ Take part in case discussions with pharmacists, nursing staff, the pharmacy support manager, and fellow pharmacy care coordinators.
β’ Execute additional related tasks as assigned.
β’ Must reside in either Texas or Kentucky.
β’ A high school diploma is required.
β’ At least two (2) years of experience in a healthcare-related call center, physician's office, or social service setting.
β’ Preferred educational background in business administration, accounting, sales, marketing, computer sciences, or similar fields via a two-year associate degree program or an equivalent combination of experience and education.
β’ Strong skills in time management and organization.
β’ Demonstrated initiative, professionalism, and a positive attitude.
β’ Attention to detail and a swift learning ability.

β’ Capability to work independently with a high level of initiative.
