This is a fully remote position, open to applicants in Kentucky, +1 more state.
β’ Respond to and resolve patient inquiries within designated time frames
β’ Recognize the physical, psychological, and social challenges associated with specific disease states
β’ Support patients with medication therapy and pharmacy care concerns
β’ Encourage adherence to prescribed medications
β’ Report any adverse events linked to medication therapy and pharmacy care
β’ Build and sustain relationships with physician practices
β’ Enhance communication among patients, payers, pharmacists, nurses, laboratories, and other healthcare stakeholders
β’ Engage in the administration of patients' pharmacy care plans
β’ Identify and resolve patient and payer issues promptly
β’ Collaborate within a team-oriented environment to assist healthcare team members
β’ Spot areas needing improvement and share suggestions with the healthcare team
β’ Keep accurate notes and documentation regarding patients' medication therapy and pharmacy care plans
β’ Highlight issues that necessitate coordination
β’ Coordinate responses and solutions with relevant internal and external stakeholders
β’ Participate in discussions with pharmacists, nursing services, the pharmacy support manager, and other pharmacy care coordinators to evaluate cases and care-plan concerns
β’ Carry out additional duties as assigned
β’ Must live in either Texas or Kentucky
β’ High school diploma is mandatory
β’ At least two (2) years of experience in a healthcare-related call center, physician's office, or social services setting
β’ Preferred educational background in business administration, accounting, sales, marketing, computer sciences, or related fields, ideally with a completed two-year associate degree or equivalent experience and education
β’ Strong organizational and time management abilities
β’ Initiative, professionalism, and a positive outlook are essential
β’ Detail-oriented; quick learner; self-driven and proactive
β’ Capability to work independently with a strong degree of initiative
β’ Proficient oral and written communication skills
β’ Ability to foster a supportive team atmosphere

β’ Strong interpersonal abilities
β’ Proficient in Microsoft Office Suite, including Word, Excel, Access, and Outlook
