This is a fully remote position, open to applicants in Kentucky, +1 more state.
• Serve as the advocate for patients to ensure adherence to prescribed drug therapies and pharmacy care plans.
• Provide courteous, professional, and high-quality service.
• Collaborate with patients’ healthcare providers, payers, and other health-related resources to coordinate pharmacy care.
• Address and resolve patient inquiries within established time frames.
• Assist patients with issues related to drug therapy and pharmacy care.
• Encourage adherence to prescribed medications.
• Report any adverse events connected to drug therapy and pharmacy care.
• Maintain relationships with physician practices and enhance communication between patients, payers, pharmacists, nurses, laboratories, and other healthcare entities.
• Take part in the administration of patients’ pharmacy care plans.
• Diagnose and resolve issues faced by patients and payers promptly.
• Support fellow healthcare team members in a collaborative environment.
• Identify areas for enhancement and share ideas with the healthcare team.
• Keep current records and documentation concerning patients’ drug therapy and pharmacy care plans.
• Coordinate responses and solutions with relevant internal and external parties.
• Engage in case discussions with pharmacists, nursing services, pharmacy support managers, and fellow pharmacy care coordinators.
• Perform additional duties as assigned.
• Must reside in Texas or Kentucky.
• High school diploma is required.
• At least two (2) years of experience in a healthcare-related call center, physician's office, or social service setting.
• Preferred educational background in business administration, accountancy, sales, marketing, computer sciences, or similar fields, having completed a two-year associate degree program or equivalent combination of experience and education.
• Strong organizational and time management skills.

• Capability to work independently with a high degree of initiative.
• Effective oral and written communication skills.

• Proficient in Microsoft Office Suite, including Word, Excel, Access, and Outlook.
• Ability to sit regularly and occasionally lift or move up to 10 pounds.
