Patient Care Associate

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial point of contact for patients or their families when they reach out to Sailor Health.

• Assist older adults and caregivers through the intake process with compassion, clarity, and professionalism.

• Provide support to patients, coordinate care, and offer emotional guidance.

• Aid new patients in navigating their path to care.

• Pair patients with the appropriate therapist.

• Ensure patients feel understood and confident throughout the intake experience.

• Take initiative and contribute to the development of essential processes within a small, agile team.

⛳️ Requirements

• Must reside in Canada.

• Possess strong empathetic communication skills.

• Exhibit active listening abilities and create a comfortable environment during challenging or emotional discussions.

• Highly organized with a keen attention to detail.

• Capable of managing multiple patients at various stages of their journey.

• Comfortable learning Google Workspace, EHR systems like Healthie, and CRMs such as HubSpot.

• Quick to learn new software tools.

• Genuine interest in mental health and improving healthcare accessibility.

• Experience in healthcare, customer service, or administrative support is advantageous but not mandatory.

• Availability to work during Eastern Time Zone hours.

• Must be willing to work one of the specified shifts: either 9 a.m.–6 p.m. ET or 12 p.m.–9 p.m. ET.
