Patient Care Associate

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the primary point of contact for patients and their families reaching out to Sailor Health.

• Assist older adults and caregivers through the intake process with compassion, clarity, and professionalism.

• Offer patient support, care coordination, and emotional assistance.

• Aid patients in navigating their entry into care, including connecting them with the appropriate therapist.

• Ensure that patients feel supported, confident, and understood throughout the intake process.

• Take initiative and contribute to the development of essential processes within a small, agile team.

⛳️ Requirements

• Located in Canada.

• Possess strong empathy and effective communication skills.

• Demonstrated active listening skills and the ability to help others feel comfortable during challenging conversations.

• Highly organized and detail-focused.

• Willingness to learn Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Capable of quickly adapting to new software.

• Interest in mental health and access to healthcare.

• Experience in healthcare, customer service, or administrative support is beneficial but not mandatory.

• Comfortable working hours aligned with the Eastern Time Zone.

• Availability to work a scheduled shift from 9 a.m. to 6 p.m. ET or 12 p.m. to 9 p.m. ET.
