Part Time Virtual Family Practice Nurse Practitioner, LA Licensed

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Assess the daily caseload and set up technology for patient services.

• Utilize modern technology to navigate the daily schedule.

• Collaborate with clinical in-home team members to visit patients in their homes.

• Engage with a diverse patient population and gather pertinent information.

• Conduct virtual wellness visits and health assessments for adults and/or pediatric patients.

• Execute health risk assessments, medication reviews, and cognitive evaluations.

• Equip patients with educational resources related to their health and wellness.

• Work with Sprinters to assess vital signs and coordinate blood draws.

• Address individual patient needs through specific tasks.

• Manage care coordination, including preventive care, medication management, specialist referrals, community resources, and accurate documentation.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Valid Family Nurse Practitioner license in Louisiana.

• Experience in conducting wellness visits.

• Demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion.

• Exhibit a willingness to tackle challenges and think creatively.

• Possess technological proficiency; comfortable with laptops, mobile devices, and HIPAA-secure messaging applications.

• Strong written and verbal communication abilities.

• Capability to work autonomously or as part of a team.

• Successfully pass a national background check and have a valid clinical license search.

• Previous experience with Medicare and/or Medicaid Annual Wellness Visits is preferred in the application.

• Medicaid enrollment is requested in the application.

• Virtual experience as a Family Nurse Practitioner is requested in the application.

• Current prescriptive authority in the states of licensure is requested in the application.
