Part-Time Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform monthly CCM phone consultations with the designated practice.

• Fulfill necessary patient interactions to achieve CCM service objectives.

• Provide education to patients regarding chronic conditions and their care plans.

• Coordinate care with healthcare provider offices, pharmacies, and community resources.

• Recognize gaps in care and social determinants affecting health.

• Develop and revise individualized care plans within the EMR system.

• Ensure precise documentation of all patient interactions.

• Uphold HIPAA compliance and maintain patient confidentiality.

• Spend the majority of the workday engaging with patients via phone, coordinating care, and documenting interactions.

• Achieve daily productivity targets within a structured remote work environment.

⛳️ Requirements

• A minimum of 1-3 years of experience as a CMA/RMA or LPN.

• Current certification or multistate licensure for LPNs.

• Excellent verbal and written communication abilities in English.

• Comfortable with spending most of the day on the phone.

• Familiarity with EMR/EHR systems.

• Organized, dependable, and capable of working independently in a remote environment.

• Preferred: Experience working remotely.

• Preferred: Experience with geriatric patients.

• Preferred: Background in care coordination or case management.

• Preferred: Proficient verbal and written communication skills in Spanish.

• Available to work 8-29 hours weekly.

• Preferred schedule between 10:00 AM-6:00 PM in the relevant time zone.

• Availability to work one weekend each month.

• Willing to work at least two holidays per year.

• A private home office that is free from distractions.

• A reliable internet connection with upload speeds of at least 100 Mbps.

• Satellite internet and mobile hotspots are not compatible with this position.
