This is a fully remote position, open to applicants in Pennsylvania, +1 more state.
• Act as the primary contact for incoming patient referrals to the Kidney Palliative Care Program.
• Perform medical record evaluations and collaborate with the care team to assess advance care planning requirements for CKD and ESKD patients.
• Offer actionable recommendations and subsequent steps to the palliative care RN or Social Worker for smooth and efficient transitions.
• Facilitate communication across the interdisciplinary team regarding patient enrollment and care strategies.
• Deliver timely education and support to clinical colleagues.
• Recognize trends and opportunities for education.
• Collaborate with leadership on quality enhancement initiatives.
• Assess complex kidney patients and connect them with suitable palliative resources.
• Contribute to the expansion of DaVita IKC’s national kidney care palliative program.
• Current Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) with an active, unencumbered license.
• Graduate from an accredited nursing or social work program is mandatory.
• A minimum of ten (10) years of experience in clinical nursing or social work is required.
• Experience in hospice or palliative care is essential.
• Renal nursing or social work experience is preferred.
• Proficient in medical record analysis, patient evaluation, and care planning.
• Capable of providing clear and consultative feedback to clinical colleagues.
• Skilled in identifying trends, resolving issues, and enhancing processes collaboratively.
• Ability to work independently and manage time effectively in a dynamic remote setting.
• Basic computer literacy and proficiency in MS Word and Outlook are required.
• Home office must have internet connectivity of at least 1MB upload and 1MB download speed.
• Availability to work Monday through Friday.
