OB RN Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee a caseload of expectant members by providing telephonic support and coordinating their care.

• Educate and assist members throughout their pregnancy, delivery, and postpartum phases.

• Perform thorough assessments and create individualized care plans.

• Arrange medical appointments, referrals, and ensure follow-up care is consistent across providers and community resources.

• Track care plans, assess progress, and implement necessary modifications.

• Guarantee that members receive the appropriate services aligned with their health requirements.

• Maintain precise documentation within insurance systems and comply with all relevant standards.

• Work collaboratively with physicians, social workers, and other members of the care team to enhance outcomes for members.

⛳️ Requirements

• A minimum of 3 years of clinical experience in Labor & Delivery is required.

• A Bachelor’s degree in a health-related discipline is preferred.

• An active, unrestricted RN license in Indiana is mandatory.

• Multi-state licensure is preferred.

• Case Management certification is preferred.

• Strong organizational skills, meticulous attention to detail, and effective time management abilities are essential.

• Excellent communication skills with a compassionate approach and a dedication to supporting expectant mothers.

• Ability to dedicate a full workday in a dynamic and fast-paced environment.
