Nurse Supervisor

This is a fully remote position, open to applicants in United States.

📋 Description

• Lead a team of Care Navigators engaged in chronic care management and/or remote patient monitoring programs.

• Cultivate engaged Care Navigator teams that foster trusting therapeutic relationships with patients.

• Supervise patient-facing activities and enhance performance optimization of clinical outreach with selected practices.

• Build supportive relationships with Care Navigators, equipping them with essential tools, skills, resources, and confidence.

• Retain exceptional talent and nurture a positive workplace atmosphere.

• Advocate for adherence to partner Workflow Strategy Guides.

• Design and deliver partner-specific workflow training.

• Collaborate with the Clinical Operations Manager and the Customer Success Team.

• Allocate patient caseloads and responsibilities to Care Navigators.

• Assist with the enrollment and unenrollment processes for programs.

• Monitor Care Navigator performance, providing coaching and support as needed.

• Regularly shadow and coach Care Navigators.

• Conduct monthly one-on-one meetings to identify workflow gaps.

• Facilitate formal coaching sessions, including Corrective Action Forms and Performance Improvement Plans.

• Perform monthly Quality Assurance reviews of calls and documentation.

• Ensure compliance with policies, procedures, legal, and regulatory requirements.

• Organize and lead weekly team meetings.

⛳️ Requirements

• A minimum of 5 years of experience in primary or specialty care practice, home care, or chronic care management (preferred).

• Experience in a supervisory or lead role, ideally in a remote environment (preferred).

• Background in remote monitoring or virtual care management (preferred).

• Certification as an LPN/LVN or higher (required).

• Proven ability to build highly motivated and engaged teams, preferably in a remote setting.

• High level of computer literacy, with comfort in learning and utilizing digital health portals and video calls.

• Strong communication and interpersonal abilities.

• Excellent time management and organizational skills.

• Charismatic personality with a passion for coaching and mentoring staff.
