Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Evaluate the medical and mental health requirements of patients

• Develop care plans that proactively address health conditions and tackle barriers to receiving care

• Ensure adherence to regulations set by local, state, and federal agencies concerning clinical services

• Prescribe appropriate medications

• Request laboratory tests, diagnostic procedures, and specialist consultations

• Monitor patients' adherence to treatment regimens and their responses

• Adjust treatment plans as necessary

• Collaborate with RN case managers as a key member of an integrated care team

• Work alongside providers, case managers, facility partners, and health plan leadership

• Implement TruHealth’s national care model in alignment with federal and state guidelines

• Utilize independent judgment to manage acute and chronic conditions, consulting with a supervising physician when appropriate

• Offer training and guidance to facility staff as needed

⛳️ Requirements

• Required to hold an FNP, AGNP, AHACNP, or PA license

• Must possess a degree from an accredited APRN or PA program

• Minimum of 3 years’ experience in clinical nursing or rehabilitation within geriatric populations

• Proficient in Electronic Health Records

• Familiarity with Microsoft applications, including Word, Outlook, and Excel

• Preferred experience in primary care or internal medicine, particularly with a focus on geriatrics

• Preference for experience in geriatric primary care

• Experience in long-term care is advantageous

• May be required to provide training and advice to facility staff
