Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Deliver tailored and coordinated healthcare to long-term care residents participating in specialized Medicare Advantage plans.

• Evaluate the medical and mental health requirements of patients.

• Formulate care plans that proactively address conditions and overcome barriers to care.

• Ensure adherence to local, state, and federal regulations concerning clinical services.

• Prescribe medications as necessary.

• Request lab tests, diagnostic procedures, and consultations.

• Track patient adherence to treatment and their responses.

• Adjust treatment plans as required.

• Collaborate with supervising physicians when appropriate.

• Work alongside RN case managers as part of a cohesive care team.

• Partner with facility stakeholders and health plan leadership to execute the national model of care.

• Potentially offer training and guidance to facility staff.

⛳️ Requirements

• Advanced practice provider: nurse practitioner (NP) or physician assistant (PA).

• FNP, AGNP, AHACNP or PA license is required.

• Graduate from an accredited APRN or PA program.

• Master’s degree or higher is necessary.

• Minimum of 3 years’ experience in clinical nursing or rehabilitation with geriatric populations.

• Background in primary care or internal medicine, ideally with a geriatrics focus.

• Proficiency with Electronic Health Records.

• Familiarity with Microsoft applications, including Word, Outlook, and Excel.

• Capacity to engage with aging populations and long-term care residents.

• Ability to independently make judgments in treating both acute and chronic conditions.

• APRN Advanced Practice RN certification/licensure is required.
