Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Evaluate the medical and mental health requirements of patients while developing proactive care plans.

• Identify and address obstacles to care, managing both acute and chronic health conditions.

• Ensure adherence to regulations set by local, state, and federal agencies regarding clinical services.

• Prescribe medications and arrange for laboratory tests, diagnostic procedures, and consultations.

• Monitor patient adherence to treatment and their responses, adjusting treatment plans as necessary.

• Collaborate with RN case managers as a member of an integrated care team.

• Work alongside providers, case managers, facility partners, and leadership from health plans.

• Implement TruHealth’s national care model while ensuring compliance with federal and state regulations.

• Provide training and guidance to facility staff as required.

⛳️ Requirements

• A valid FNP, AGNP, AHACNP, or PA license is required.

• Must hold a degree from an accredited APRN or PA program.

• A minimum of 3 years of experience in clinical nursing or rehabilitation with geriatric populations.

• Proficiency in Electronic Health Records is necessary.

• Familiarity with Microsoft applications, including Word, Outlook, and Excel is required.

• Experience in primary care or internal medicine is preferred, ideally with a focus on geriatrics.

• Qualification as an advanced practice registered nurse (APRN or NP) or physician assistant (PA) is essential.

• Experience in Primary Care within Geriatric Medicine is preferred.

• Long-term care experience is advantageous.

• An APRN Advanced Practice RN certification/license is mandatory.
