Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Deliver tailored and coordinated health care to residents in long-term care who are enrolled in specific Medicare Advantage plans.

• Utilize independent judgment to manage patients with both acute and chronic health conditions.

• Consult with a supervising physician when necessary.

• Collaborate effectively with providers, RN case managers, facility partners, and health plan leadership.

• Implement the national care model while adhering to federal and state regulations.

• Evaluate the medical and mental health requirements of patients.

• Create care plans that proactively manage health conditions and address obstacles to care.

• Ensure adherence to the guidelines set by local, state, and federal agencies.

• Prescribe medications and order lab tests, diagnostic procedures, and consultations.

• Monitor patient compliance and treatment effectiveness, adjusting treatment plans as necessary.

• Provide training and guidance to facility staff as needed.

⛳️ Requirements

• A valid FNP, AGNP, AHACNP, or PA license is mandatory.

• An accredited degree from an APRN or PA program is required.

• A minimum of 3 years of experience in clinical nursing or rehabilitation within geriatric populations.

• Proficiency in Electronic Health Records is essential.

• Familiarity with Microsoft applications, including Word, Outlook, and Excel.

• Prior experience in primary care or internal medicine is preferred, especially with a focus on geriatrics.

• A Master's degree or higher is desirable.

• Certification/license as an APRN Advanced Practice RN is required.

• Experience in Primary Care within Geriatric Medicine is preferred.

• Experience in Long-Term Care is also preferred.
