Nurse Practitioner, Weekend On-Call

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Evaluate the medical and mental health needs of patients and develop proactive care plans.

• Ensure adherence to clinical service regulations set by local, state, and federal agencies.

• Prescribe medications and request laboratory tests, diagnostic procedures, and specialist consultations.

• Monitor patient adherence to treatment and responses, adjusting treatment plans as necessary.

• Collaborate with RN case managers within an integrated care team framework.

• Provide care for patients with both acute and chronic conditions, exercising independent judgment while consulting a supervising physician when appropriate.

• Partner with facility collaborators and health plan executives to implement the national care model.

• Offer training and guidance to facility personnel as needed.

• Enhance outcomes and quality of life for long-term care residents while minimizing avoidable hospitalizations.

⛳️ Requirements

• FNP, AGNP, AHACNP, or PA license is mandatory.

• Must possess a degree from an accredited APRN or PA program.

• A Master's degree or higher is required.

• Minimum of 3 years of experience in clinical nursing or rehabilitation with geriatric populations.

• Experience in primary care or internal medicine, ideally with a focus on geriatrics.

• Familiarity with Electronic Health Records is essential.

• Proficient in Microsoft applications, including Word, Outlook, and Excel.

• Preferred experience in primary care within geriatric medicine.

• Previous long-term care experience is a plus.

• Capability to adhere to federal, state, and local regulations is necessary.

• APRN Advanced Practice RN certification/license is required.
