Nurse Practitioner, SC License Required

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Address care needs within the Transitions of Care program.

• Engage in telephonic outreach and conduct scheduled video or phone consultations for patients who have recently been hospitalized and discharged.

• Execute medication reconciliation and perform symptom assessments.

• Request necessary follow-up labs, medications, or supplies.

• Establish trust, elucidate Galileo's care model, and link patients to available services.

• Coordinate care among hospitals, emergency departments, pharmacies, and community providers.

• Collaborate effectively with the nursing team.

• Achieve individual performance objectives while also contributing to team-focused population health initiatives.

• Work 4-hour shifts in mornings, afternoons, evenings, or overnight periods during both weekdays and weekends.

• Provide primary care and transition-of-care follow-up as part of a 24/7 medical group.

⛳️ Requirements

• Certified Nurse Practitioner in Adult, Family, or Geriatrics.

• Active state licensure in South Carolina, with the capacity to secure additional state licenses.

• Over 5 years of post-graduate clinical experience in Primary Care or Geriatrics.

• Up to 2 years of experience may be compensated by time spent working in a Skilled-Nursing or Long-Term Care facility.

• Direct experience in engaging Medicare and Medicaid populations while managing complex, comorbid conditions.

• Proficiency in motivational interviewing techniques.

• Comfortable in a digital-first setting, with experience in or a strong enthusiasm for virtual care delivery.

• Strong technological skills; familiarity with Google Suite and Slack is advantageous.

• Exceptional written and verbal communication abilities.

• Bilingual proficiency in English and Spanish, including cultural fluency, is a significant asset.
