This is a fully remote position, open to applicants in United States.
β’ Perform comprehensive cardiovascular assessments for patients via telehealth, including review of medical history, physical examinations, and diagnostic evaluations.
β’ Develop differential diagnoses and treatment strategies; prescribe medications and request diagnostic tests.
β’ Manage conditions such as hypertension, congestive heart failure, arrhythmias, and coronary artery disease remotely.
β’ Provide education to patients and their families regarding cardiovascular health, prevention strategies, medication adherence, dietary recommendations, exercise, and smoking cessation.
β’ Collaborate with cardiologists, primary care providers, advanced practice providers (APPs), and other healthcare team members to ensure coordinated patient care.
β’ Integrate the latest cardiology research and evidence-based methods into patient care.
β’ Keep accurate patient records while ensuring compliance with legal and ethical standards.
β’ Advocate for patients and their preferences regarding care.
β’ Engage in continuing education and quality assurance initiatives.
β’ Lead and support clinical quality projects, including peer reviews and oversight for APP and physician quality.
β’ Enhance and develop clinical quality programs and performance metrics.
β’ Create and implement onboarding, training, and ongoing educational programs for clinicians.
β’ Conduct chart reviews, offer performance evaluations, and encourage adherence to evidence-based practices and organizational protocols.
β’ Act as a clinical resource and mentor to promote excellence in patient care and support clinician development.
β’ Masterβs or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
β’ Current, unrestricted Nurse Practitioner/APRN license.
β’ Active national board certification, such as AANP or ANCC.
β’ Eligibility to obtain and maintain licensure in states as required by the organization.
β’ Extensive clinical experience as a Nurse Practitioner managing cardiovascular conditions, including hypertension, heart failure, arrhythmias, and coronary artery disease.
β’ Preferred experience in cardiology.
β’ Capability to independently assess patients, form differential diagnoses, devise evidence-based treatment plans, prescribe medications, and order and interpret diagnostic tests.
β’ Experience with clinical quality initiatives, chart reviews, peer reviews, quality assurance, or performance improvement programs.
β’ Background in developing or providing clinical education, onboarding, training, or professional development for clinicians.
β’ Ability to offer constructive feedback, mentor peers, and enhance clinician performance.
β’ Capacity to serve as a clinical resource and mentor for APPs and other clinicians.
β’ Strong understanding of evidence-based cardiovascular care along with current clinical guidelines and research.
β’ Comfortable delivering care via telehealth and utilizing virtual care technology.
β’ Exceptional written and verbal communication skills along with proficiency in cross-functional collaboration.
β’ Dedication to patient advocacy, education, shared decision-making, continuing education, and maintaining necessary credentials.
