Nurse Practitioner, PRN

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Conduct annual risk evaluations in residential environments or through telehealth services within the Nurse Practitioner scope, as assigned by the Collaborative Physician.

• Execute thorough in-home health risk assessments to identify both active and chronic health conditions and assess physical, mental, and social needs.

• Gather patient history, carry out examinations, and establish diagnoses.

• Provide written documentation of patient visits in accordance with NCQA standards.

• Measure patient vital signs as required.

• Initiate case management referrals and liaise with Primary Care Providers (PCPs) as needed.

• Communicate effectively with patients, caregivers, agency nurses, other healthcare providers, and vendors to ensure accurate diagnosis.

• Carry out clinical responsibilities while adhering to OSHA Universal Precautions.

• Uphold patient confidentiality at all times.

• Attend mandatory meetings and in-services, and participate in committees as needed.

• Engage in professional development opportunities and maintain necessary professional licenses and affiliations.

⛳️ Requirements

• Active and unrestricted nurse practitioner license valid in coverage states.

• Board certification from one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).

• Current Basic Life Support (BLS) Certification.

• Active enrollment in Medicare/Medicaid programs.

• Must possess a valid driver’s license and maintain a good driving record.

• Excellent electronic health record (EHR) skills.

• Geriatric training or experience is preferred.

• Ability to adapt to varying weather conditions and patients’ home/residential settings.

• Full range of motion, including the ability to handle and lift patients; manual dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking, and stair climbing capabilities.

• Regularly lift and carry items weighing up to 50 pounds.

• Capability to travel by automobile or van up to 150 miles daily in urban and/or rural areas; ability to drive if necessary.
