This is a fully remote position, open to applicants in Alaska, +5 more states.
β’ Perform telehealth clinical assessments to evaluate both acute and chronic health requirements, preventive care needs, and social determinants of health.
β’ Assess the clinical suitability for Community Health Worker services and provide referrals for care coordination and community-based assistance.
β’ Create personalized care plans that address clinical requirements and social determinant factors.
β’ Collaborate with Community Health Workers to enhance patient engagement and ensure follow-through on care plans.
β’ Manage acute, chronic, preventive, and medication management needs within the program's scope.
β’ Prescribe and renew non-controlled medications when clinically warranted.
β’ Accurately document visits in a timely manner in accordance with clinical, state, federal, and program standards.
β’ Assist patients in navigating healthcare systems and connecting with medical and community resources.
β’ Choose shifts based on availability and patient demand during the PRN phase.
β’ Progress toward a stable schedule or full-time position as patient volume increases.
β’ An active, unrestricted Nurse Practitioner/APRN license along with all necessary prescribing or furnishing authority required for practice in the relevant state(s).
β’ Must hold a license in at least one of the following states: Alaska, Arizona, California, Louisiana, New Mexico, or Nevada.
β’ Licensure in multiple states is highly preferred, with a willingness to obtain additional state licenses as necessary.
β’ Experience in primary care, family medicine, internal medicine, urgent care, or chronic disease management is strongly preferred.
β’ Telehealth experience is advantageous.
β’ Familiarity with working with Medicaid and Medicare populations.
β’ Strong comprehension of social determinants of health and their influence on patient outcomes.
β’ Comfortable using telehealth platforms and establishing rapport in a virtual setting.
β’ Exceptional communication skills with the ability to engage patients from various backgrounds.
β’ Knowledge of state, federal, and payer requirements relevant to CHW services and referrals, or a willingness to learn.
β’ Fluency in Spanish or other languages is strongly preferred.
β’ No guaranteed minimum number of hours per week during the PRN phase.
β’ Flexible scheduling with no minimum hours guarantee.
