Nurse Practitioner – National After-Hours Team, Part Time, IA Licensed

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Act as the primary support for residents and facility staff during after-hours.

• Offer guidance and medical assistance via phone communications.

• Utilize Curana’s telephonic platform for making and receiving calls.

• Facilitate coordination of care among facilities, hospitals, and clinics.

• Provide high-quality, cost-efficient care for acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.

• Conduct at least 30% of telehealth visits, which include evaluations for acute changes, falls, and controlled substance consultations.

• Handle high volumes of calls, managing multiple inquiries independently across several states.

• Execute comprehensive assessments.

• Accurately and thoroughly document encounters in the EMR, adhering to CMS requirements.

• Implement clinical protocols and practice guidelines to ensure safe and effective treatment in situ whenever feasible.

• Provide coverage during weeknights, weekends, and holidays.

⛳️ Requirements

• Master’s Degree as a Nurse Practitioner.

• Current unrestricted NP license in Iowa, Missouri, or Ohio; Iowa licensure is necessary for consideration.

• Capability to secure additional multi-state licenses, including Illinois, Kansas, Indiana, Minnesota, Nebraska, and Wisconsin.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP).

• Ability to obtain DEA licensure and Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical expertise in adult, family, or geriatric care.

• A minimum of 3 years of experience working as a Nurse Practitioner.

• Capacity to secure a collaborative practice agreement, if required in relevant state(s).

• Willingness to work scheduled shifts in line with scheduling policies.

• Proficient in computer skills, including the documentation of medical information through paper and electronic medical records.

• Availability for overnight and holiday shifts is essential.

• Requirement to complete two 12-hour holiday shifts each year.

• Preferred experience working in a nursing home or with seniors in an acute care setting.

• Preferred understanding of geriatrics, chronic illnesses, acute disease management, advanced illness, and discussions related to end-of-life care.
