Nurse Practitioner – National After-Hours Team – Part Time

This is a fully remote position, open to applicants in New York, +2 more states.

📋 Description

• Act as the primary support for residents and facility staff during after-hours.

• Offer guidance and medical assistance over the phone.

• Utilize Curana’s telephonic platform for initiating and receiving calls.

• Facilitate care coordination among facilities, hospitals, and clinics.

• Provide high-quality, cost-efficient care that addresses acute, chronic, and behavioral health needs in partnership with physicians and specialty providers.

• Conduct thorough assessments and accurately document encounters in the EMR to ensure compliance with CMS requirements.

• Implement Curana’s clinical protocols and practice guidelines to support in-place treatment whenever feasible.

• Deliver empathetic telephonic and virtual care, which includes assessing acute changes, falls, and controlled substance visits.

• Engage in mandatory education and training sessions.

• Cover weeknight, weekend, overnight, and holiday shifts as per the established schedule.

• Provide coverage every other weekend on Saturday and Sunday for 12-hour day shifts.

• Work two holidays annually as a part-time team member.

⛳️ Requirements

• Master's Degree in Nursing as a Nurse Practitioner.

• Current unrestricted NP license in Pennsylvania and New York and/or Michigan is required; licensure in at least two of these states is necessary.

• Must be active or willing to secure licensure within 30 days for the District of Columbia, New York, Maryland, Virginia, and West Virginia.

• National Nurse Practitioner certification as ANP, FNP, or GNP.

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical experience in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Ability to establish a collaborative practice agreement, if applicable in your state(s).

• Flexibility to work scheduled shifts in accordance with scheduling policies.

• Proficient in computer skills, including documentation of medical information in both written and electronic medical records.

• Comfortable handling high call volumes.

• Ability to conduct at least 30% of visits via telehealth.

• Skillful in managing multiple calls independently while providing care across various states.
