Nurse Practitioner – National After-Hours Team, Part Time

This is a fully remote position, open to applicants in Louisiana, +2 more states.

📋 Description

• Act as the primary support for residents and facility staff during after-hours.

• Offer guidance and medical assistance via phone.

• Utilize Curana’s telephonic platform for making and receiving calls.

• Facilitate care coordination among facilities, hospitals, and clinics.

• Provide care that addresses acute, chronic, and behavioral health needs in partnership with physicians and specialty providers.

• Conduct thorough assessments and properly document interactions in the EMR.

• Ensure compliance with CMS regulations.

• Implement clinical protocols and practice guidelines to support treatment in situ when feasible.

• Handle high call volumes and multiple calls independently.

• Conduct telehealth visits, including evaluations of acute changes, falls, and controlled substance consultations.

• Offer weeknight coverage from 5pm to 8am local time.

• Provide continuous weekend coverage from Friday at 5pm to Monday at 8am.

• Cover every other weekend with 12-hour shifts on Saturday and Sunday or mid-shifts.

• Deliver overnight and holiday coverage.

⛳️ Requirements

• Master's Degree as a Nurse Practitioner.

• Active and unrestricted licensure in Texas and Louisiana and/or Tennessee, or another approved state.

• National Nurse Practitioner certification as ANP, FNP, or GNP.

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical background in adult, family, or geriatric care.

• Over 3 years of experience as a Nurse Practitioner.

• Ability to secure a collaborative practice agreement, if required in your state(s).

• Willingness to work scheduled shifts following scheduling policies.

• Proficient in computer skills, including documentation of medical information in both written and electronic medical records.

• Experience in a nursing home or with elderly patients in an acute care setting.

• Knowledge of geriatrics, chronic illnesses, and acute disease management.

• Familiarity with advanced illness and end-of-life conversation techniques.

• Strong ability to develop and maintain positive relationships with customers.

• Adaptable to change.

• Must be available for overnight and holiday shifts; two holidays per year are mandatory.

• Required to perform at least 30% of telehealth visits.
