Nurse Practitioner – National After-Hours Team, Part Time

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Offer after-hours telephonic care and clinical guidance to residents and facility personnel.

• Act as the primary support during evenings, nights, weekends, and holidays.

• Utilize Curana’s telephonic platform to manage incoming and outgoing calls.

• Facilitate coordination of care among facilities, hospitals, and clinics.

• Provide care for acute, chronic, and behavioral health requirements in collaboration with physicians and specialty providers.

• Conduct thorough assessments and accurately document interactions in the EMR, adhering to CMS standards.

• Assess acute changes, falls, and controlled substance visits using telehealth services.

• Implement clinical protocols and practice guidelines to support in-place treatment whenever feasible.

• Engage in mandatory training and educational programs.

• Work two weeknight shifts from 8pm to 2am.

• Cover every other weekend with 12-hour night shifts on Saturday and Sunday, from 6pm to 6am.

• Ensure necessary overnight and holiday coverage.

⛳️ Requirements

• Master’s Degree as a Nurse Practitioner.

• Current licensure in North Carolina and South Carolina is mandatory.

• Additional active licenses in Florida and Georgia are highly preferred.

• Willingness to obtain supplementary state licenses within a 30-day period is required.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP).

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical expertise in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Ability to secure a collaborative practice agreement, if applicable in your state(s).

• Availability to work scheduled shifts according to scheduling policies.

• Proficient in computer skills, including documentation of medical information in both written and electronic medical records.

• Experience in a nursing home or with seniors in an acute care environment is preferred.

• Knowledge of geriatrics, chronic illnesses, and acute disease management is advantageous.

• Familiarity with advanced illness and end-of-life discussions is preferred.

• Strong ability to foster and maintain positive customer relationships.

• Flexibility and adaptability to change.

• Must be capable of handling high call volumes and managing multiple calls independently.

• At least 30% of visits must be conducted via telehealth.

• Must be available for overnight and holiday shifts, including two holidays per year.

• Visa sponsorship is not available.
