Nurse Practitioner – National After-Hours Team – Part Time

This is a fully remote position, open to applicants in District of Columbia, +6 more states.

📋 Description

• Act as the primary support for residents and facility personnel during after-hours, offering guidance and medical assistance via phone.

• Utilize Curana’s telephonic platform for making and receiving calls, facilitating care coordination among facilities, hospitals, and clinics.

• Provide high-quality, cost-efficient care that addresses acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.

• Conduct thorough assessments and meticulously document interactions in the EMR, ensuring compliance with CMS regulations.

• Implement clinical protocols and practice guidelines to ensure safe and effective treatment is delivered whenever feasible.

• Deliver virtual care mainly by phone to elderly residents across various states.

• Conduct telehealth consultations focusing on acute changes, falls, and controlled substance visits.

• Cover shifts during weeknights, weekends, overnight, and holidays as required.

• Engage in mandatory training and education programs.

⛳️ Requirements

• Master’s Degree in Nursing as a Nurse Practitioner.

• Current unrestricted Nurse Practitioner license in Pennsylvania and New York and/or Michigan is required; licensure in at least two of these states is necessary.

• Must be active or willing to obtain licensure within 30 days for the District of Columbia, New York, Maryland, Virginia, and West Virginia.

• National certification as a Nurse Practitioner in ANP, FNP, or GNP is required.

• Eligibility to secure DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical experience in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Capability to obtain a collaborative practice agreement, if necessary in your state(s).

• Availability to work scheduled shifts in alignment with scheduling policies.

• Proficient in computer skills, including documenting medical information in written and electronic medical records.

• Preferred experience working in a nursing home or with seniors in an acute care setting.

• Familiarity with geriatrics, chronic illness, and acute disease management is preferred.

• Knowledge of advanced illness and end-of-life discussions is preferred.

• Ability to cultivate and maintain positive customer relationships is preferred.

• Flexibility and adaptability to change is preferred.

• Must effectively manage high call volumes and multiple calls independently.

• Minimum of 30% of visits must be conducted via telehealth.

• Availability for overnight and holiday shifts is required; part-time employees must cover two holidays per year.
