Nurse Practitioner – National After-Hours Team, Part Time

This is a fully remote position, open to applicants in District of Columbia, +6 more states.

📋 Description

• Act as the primary support for residents and facility staff during after-hours, offering guidance and medical assistance over the phone.

• Utilize Curana’s telephonic platform for making and receiving calls, facilitating care coordination among facilities, hospitals, and clinics.

• Provide high-quality, cost-effective care that addresses acute, chronic, and behavioral health needs in partnership with physicians and specialty providers.

• Conduct thorough assessments and accurately document encounters in the EMR, ensuring adherence to CMS requirements.

• Implement Curana’s clinical protocols and practice guidelines to ensure safe and effective treatment is administered in place whenever feasible.

• Engage in required education and training to remain updated with care standards.

• Offer telephonic and telehealth services during evenings, nights, weekends, and holidays.

• Handle high call volumes and multiple calls independently.

• Conduct at least 30% of telehealth visits, including evaluations for acute changes, falls, and controlled substance visits.

• Deliver care across various states.

• Cover weekday nights, weekends, overnight shifts, and required holidays.

⛳️ Requirements

• Master’s Degree as a Nurse Practitioner.

• Current unrestricted NP license in Pennsylvania and New York and/or Michigan is mandatory; must hold a license in at least two of these states.

• Active or willingness to obtain licensure within 30 days for the District of Columbia, New York, Maryland, Virginia, and West Virginia is required.

• National certification as Nurse Practitioner (ANP, FNP, or GNP).

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical expertise in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Ability to establish a collaborative practice agreement, if applicable in your state(s).

• Availability to work scheduled shifts in accordance with scheduling policies.

• Proficient computer skills, including the ability to document medical information using written and electronic medical records.

• Preference for candidates with experience in a nursing home or with seniors in an acute care setting.

• Familiarity with Geriatrics, Chronic Illness, and acute disease management is preferred.

• Understanding of Advanced Illness and end-of-life discussions is preferred.

• Ability to build and maintain positive customer relationships is preferred.

• Adaptability to change is preferred.

• The company is currently unable to provide visa sponsorship (H1B or otherwise).
