Nurse Practitioner – National After-Hours Team

This is a fully remote position, open to applicants in New York, +5 more states.

📋 Description

• Act as the primary support for residents and facility personnel during after-hours.

• Offer guidance and medical assistance over the phone.

• Utilize Curana’s telephonic platform to manage calls effectively.

• Facilitate coordination of care among facilities, hospitals, and clinics.

• Provide care for acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.

• Conduct comprehensive assessments and accurately document encounters in the EMR.

• Ensure adherence to CMS standards.

• Implement clinical protocols and practice guidelines to support treatment in situ whenever feasible.

• Conduct at least 30% of visits via telehealth, including evaluations for acute changes, falls, and controlled substance visits.

• Engage in mandatory education and training sessions.

• Cover shifts during weeknights, weekends, overnight, and holidays after-hours.

• Work either two six-hour shifts on weeknights or one twelve-hour weeknight shift.

• Provide 12-hour day-shift coverage every other weekend, Saturday and Sunday.

⛳️ Requirements

• Master's Degree in Nursing as a Nurse Practitioner.

• Current, unrestricted NP license in Pennsylvania and New York and/or Michigan is required; must hold a license in at least two of these states.

• Must be active or willing to obtain licensure within 30 days for the District of Columbia, New York, Maryland, Virginia, and West Virginia.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP) is required.

• Ability to acquire DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical experience in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Capability to establish a collaborative practice agreement, if necessary in your state(s).

• Must be able to work scheduled shifts in compliance with scheduling policies.

• Proficient in computer skills, including the documentation of medical information in both written and electronic medical records.

• Preferred experience in nursing homes or acute care facilities with seniors.

• Preferred understanding of geriatrics, chronic illness, and acute disease management.

• Preferred knowledge of advanced illness and end-of-life conversations.

• Preferred ability to foster and maintain positive customer relationships.

• Preferred adaptability to changing conditions.

• Availability for overnight and holiday shifts is required; part-time team members must cover two holidays annually.

• Must be capable of managing high call volumes and conducting at least 30% of visits via telehealth.

• Must be able to handle multiple calls independently while providing care across several states.

• The company cannot provide visa sponsorship.
