Nurse Practitioner – National After-Hours Team

This is a fully remote position, open to applicants in Louisiana, +2 more states.

📋 Description

• Act as the primary support contact for residents and facility staff during after-hours.

• Offer guidance and medical assistance via phone.

• Utilize Curana’s telephonic platform for making and receiving calls.

• Facilitate coordination of care among facilities, hospitals, and clinics.

• Provide cost-effective solutions to address acute, chronic, and behavioral health needs in partnership with physicians and specialized providers.

• Conduct a minimum of 30% of telehealth visits, including assessments for acute changes, falls, and controlled substance consultations.

• Execute thorough comprehensive assessments.

• Accurately and meticulously document encounters in the EMR, adhering to CMS guidelines.

• Implement clinical protocols and practice guidelines to enable treatment in place whenever feasible.

• Engage in mandatory education and training.

⛳️ Requirements

• Master's Degree in Nursing as a Nurse Practitioner.

• Active and unrestricted licensure in Texas, Louisiana, Tennessee, or another designated state is mandatory.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP) is required.

• Must be able to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical experience in adult, family, or geriatric care.

• Minimum of 3 years of experience as a Nurse Practitioner.

• Capability to secure a collaborative practice agreement if necessary in your state(s).

• Willingness to work scheduled shifts in line with scheduling policies.

• Strong computer skills, particularly in documenting medical information using both written and electronic medical records.

• Prior experience in a nursing home or with seniors in an acute care setting.

• Knowledge of geriatrics, chronic illnesses, and acute disease management.

• Familiarity with advanced illness and end-of-life discussions.

• Proficient in building and maintaining positive relationships with customers.

• Flexibility in adapting to change.

• Availability for overnight and holiday shifts is required.

• Commitment to working two holidays per year is necessary.

• Ability to handle high call volumes and conduct at least 30% of telehealth visits.

• Skill in managing multiple calls independently while providing care across various states.
