Nurse Practitioner – National After-Hours Team

This is a fully remote position, open to applicants in Illinois, +7 more states.

📋 Description

• Provide empathetic telephonic care and clinical guidance during evenings, nights, weekends, and holidays

• Act as the primary support for residents and facility staff during after-hours

• Offer medical direction and care via phone

• Utilize Curana’s telephonic platform for making and receiving calls

• Coordinate care among facilities, hospitals, and clinics

• Deliver high-quality, cost-efficient care that addresses acute, chronic, and behavioral health needs

• Collaborate with physicians and specialty providers

• Conduct at least 30% of telehealth visits, including assessments of acute changes, falls, and controlled substance visits

• Manage high call volumes and multiple calls independently across various states

• Perform comprehensive assessments and accurately document encounters in the EMR

• Ensure adherence to CMS requirements

• Implement clinical protocols and practice guidelines to support safe treatment in place whenever feasible

• Participate in required education and training

• Provide weeknight coverage from 5pm–8am local time

• Offer continuous weekend coverage from Friday at 5pm to Monday at 8am

• Supply holiday coverage from 5pm at the end of the last business day until 8am when business hours resume

• Maintain a consistent schedule including weeknight 12-hour or 6-hour night shifts and every-other-weekend 12-hour day shifts

• Work overnight shifts and complete four 12-hour holiday shifts each year

⛳️ Requirements

• Master's Degree as a Nurse Practitioner

• Current unrestricted NP license in Iowa, Missouri, or Ohio; must hold licenses in at least 2 of these states for consideration

• Capability to obtain additional multi-state licenses, including but not limited to Illinois, Kansas, Indiana, Minnesota, Nebraska, and Wisconsin

• National certification as a Nurse Practitioner (ANP, FNP, or GNP)

• Ability to secure DEA licensure / Prescriptive Authority

• Experience in managing acute and chronic diseases

• Clinical experience in adult, family, or geriatric care

• 3+ years of experience as a Nurse Practitioner

• Ability to establish a collaborative practice agreement, if required in the applicable state(s)

• Capacity to work scheduled shifts in accordance with scheduling policies

• Proficient computer skills, including the ability to document medical information in both written and electronic medical records

• Experience working in a nursing home or with seniors in an acute care setting (preferred)

• Familiarity with geriatrics, chronic illness, and acute disease management (preferred)

• Understanding of advanced illness and end-of-life discussions (preferred)

• Ability to foster and maintain positive customer relationships (preferred)

• Adaptability to change (preferred)
