Nurse Practitioner – National After-Hours Team

This is a fully remote position, open to applicants in Florida, +2 more states.

📋 Description

• Offer after-hours telephonic guidance and medical assistance to residents and facility personnel

• Act as the primary support during evenings, nights, weekends, and holidays

• Utilize Curana’s telephonic platform for making and receiving calls

• Coordinate care among facilities, hospitals, and clinics

• Provide care for acute, chronic, and behavioral health issues in partnership with physicians and specialty providers

• Conduct a minimum of 30% telehealth visits, including assessments of acute changes, falls, and controlled substance visits

• Execute comprehensive assessments and correctly document encounters in the EMR following CMS regulations

• Implement clinical protocols and practice guidelines to facilitate treatment in place whenever feasible

• Engage in mandatory education and training sessions

• Commit to a 24-hour work week on a fixed schedule, which includes two weeknight shifts from 8pm–2am and 12-hour night shifts every other weekend on Saturday and Sunday

• Provide overnight and holiday coverage for skilled nursing and senior living facilities

⛳️ Requirements

• Master's Degree as a Nurse Practitioner

• Active licensure in North Carolina and South Carolina is required

• Additional active licenses in Florida and Georgia are strongly preferred

• Willingness to obtain extra state licenses as necessary within a 30-day period

• National Nurse Practitioner certification as ANP, FNP, or GNP

• Capability to obtain DEA licensure / Prescriptive Authority

• Background in managing acute and chronic diseases

• Clinical experience in adult, family, or geriatric care

• Over 3 years of experience as a Nurse Practitioner

• Ability to secure a collaborative practice agreement, if applicable in relevant state(s)

• Ability to adhere to scheduled shifts according to scheduling policies

• Proficient in computer skills, including documentation of medical information in written and electronic medical records

• Preferred: experience in a nursing home or with seniors in an acute care environment

• Preferred: knowledge of geriatrics, chronic illnesses, and acute disease management

• Preferred: understanding of advanced illness and end-of-life conversations

• Preferred: aptitude for developing and maintaining positive customer relationships

• Preferred: flexibility to adapt to change

• Must be capable of managing high call volumes and multiple calls independently

• Must perform at least 30% telehealth visits

• Must provide overnight and holiday coverage, including two holidays each year
