Nurse Practitioner – Full Time

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Perform virtual evaluations of patients suspected of having dementia, which includes conducting baseline cognitive assessments and ordering laboratory tests as necessary.

• Review medications and recommend the reduction of high-risk medications or the initiation of symptom-management medications as appropriate.

• Collaborate with care navigators to create detailed care plans.

• Coordinate care efforts with care navigators, primary care teams, external providers, and specialists.

• Provide support for urgent care and various geriatric practice requirements as the practice evolves.

• Engage in multidisciplinary huddles and observe care navigators and the Head of Medical.

• Offer guidance to care navigators regarding patients' clinical needs.

• Assist in the onboarding of new colleagues and work towards enhancing and scaling clinical programs.

⛳️ Requirements

• Family or adult-gerontology nurse practitioner with a minimum of 1 year of experience.

• Must be licensed in Illinois.

• Capability to obtain licensure to practice virtual/outpatient care in any state.

• Must not be excluded from participating in any state or federal healthcare programs.

• A strong passion for providing care to patients with dementia.

• Willingness to learn new technology systems.

• Comfort with working from home as part of a fully remote team.

• Familiarity with electronic medical record systems, including Athena Health.
