
Nurse Practitioner, Clinical Triage

This is a fully remote position, open to applicants in Virginia.
β’ Serve as a clinical resource for triage teams handling patients who need further clinical assessment.
β’ Assess and manage patients through telehealth to meet immediate healthcare needs.
β’ Act as a liaison between central triage resources and local clinical teams, providing updates on patients and coordinating follow-up appointments.
β’ Handle urgent medication refill requests.
β’ Manage critical lab results and other patient outcomes while ensuring coordinated care with local teams.
β’ Communicate with primary care providers for patients needing further escalation or evaluation in the Emergency Room.
β’ Respond to urgent task messages that require additional clinical assistance.
β’ Support service recovery for patients in need of prompt care.
β’ Collaborate closely with the pharmacy team to ensure comprehensive patient care.
β’ Authorize home health, durable medical equipment, death certificates, and other necessary orders.
β’ Analyze quality metrics and pod-level performance to identify areas for improvement in HEDIS performance.
β’ Educate patients about the Urgent Care Triage program aimed at reducing hospital readmissions and enhancing outcomes and satisfaction.
β’ Identify care gaps and arrange external resources, including home health, hospice, and ancillary services.
β’ Partner with RCMO and the Medical Director to explore opportunities for care improvement.
β’ Undertake additional responsibilities as assigned by the Regional CMO and/or Medical Director pertaining to clinical pod performance and outcomes.
β’ Must hold a valid Nurse Practitioner certification with active licenses in the relevant states, INCLUDING VIRGINIA.
β’ Must possess an active DEA license.
β’ Exceptional written and verbal communication skills.
β’ Capable of making quick and decisive actions in urgent and emergency situations.
β’ Proven ability to juggle multiple tasks effectively.
β’ Strong communication abilities, both written and spoken.
β’ Excellent organizational skills with a keen attention to detail.
β’ Familiarity with APRIMA EMR is preferred.
β’ Experience in telehealth and virtual care coordination is preferred.
β’ Multistate License (MSL) is preferred.
β’ A background check in Florida may be necessary as part of the hiring process.
