
Nurse Practitioner, Clinical Triage

This is a fully remote position, open to applicants in Ohio.
• Serve as a clinical resource for triage teams assisting patients who need further clinical assessment.
• Assess and treat patients through telehealth services to meet urgent health requirements.
• Act as a liaison between central triage resources and local clinical teams, providing patient updates and organizing follow-up appointments.
• Manage time-sensitive requests for medication refills, lab results, and other patient communications.
• Coordinate escalation or Emergency Room evaluations in conjunction with patients’ primary care providers.
• Support service recovery initiatives for patients needing prompt care.
• Collaborate closely with the pharmacy team to ensure comprehensive patient care.
• Sign home health documents, durable medical equipment orders, death certificates, and other necessary orders.
• Analyze quality metrics and pod-level performance, pinpoint areas for improvement, and enhance HEDIS performance.
• Provide education to patients regarding the Urgent Care Triage program.
• Identify care gaps and facilitate access to external resources such as home health, hospice, and ancillary services.
• Work with the RCMO and Medical Director on opportunities for care enhancement.
• Undertake additional responsibilities as assigned by the Regional CMO and/or Medical Director.
• Must hold a Nurse Practitioner certification with active licenses in the states covered, including Ohio.
• Must possess an active DEA license.
• Exceptional written and verbal communication abilities.
• Capable of making prompt and decisive decisions in urgent and emergency situations.
• Proven ability to manage multiple tasks simultaneously.
• Strong communication skills, both written and verbal.
• Excellent organizational skills and meticulous attention to detail.
• Experience with telehealth and virtual care coordination (preferred).
• Familiarity with APRIMA EMR (preferred).
• Multistate License (MSL) is preferred.
• At least 1 year of experience with HarmonyCares Medical Group is preferred.
• Candidates may be required to undergo a background check in the state of Florida.
