This is a fully remote position, open to applicants in Arizona, +2 more states.
β’ Act as the main clinical contact for patients as they navigate their remote care experience.
β’ Direct clinical decision-making with a focus on guideline-directed medical therapy for heart failure and evidence-based management of type 2 diabetes and hypertension.
β’ Proactively and asynchronously adjust medications based on patient vitals, symptoms, and trends.
β’ Oversee a panel of patients receiving remote care services.
β’ Supervise patient management, handle clinical escalations, and collaborate with physicians or providers to enhance treatment plans.
β’ Initiate and adjust GDMT in partnership with physicians, RNs, and care teams.
β’ Address remote patient monitoring escalations related to abnormal vitals, symptoms, and lab results.
β’ Deliver both synchronous and asynchronous care, which includes reviewing patient-generated health data, responding to monitoring alerts, secure messaging, and scheduled virtual consultations.
β’ Fulfill productivity and documentation standards.
β’ Adapt to changing workflows, software upgrades, and operational procedures.
β’ Navigate multiple technology platforms and provide insights for enhancing clinical operations and technology.
β’ Collaborate across clinical and operational teams to enhance patient care, efficiency, and scalability.
β’ Contribute to the development of improved processes within a growing digital health organization.
β’ Current Compact Multi-State RN license.
β’ Valid ANCC or AANP certification.
β’ Masterβs Degree as a Nurse Practitioner.
β’ Preferred active NP licensure in CA, WA, or AZ.
β’ Ability to consistently work Pacific Time hours, including later weekday coverage as necessary.
β’ Over 5 years of experience in managing patients with chronic diseases (CHF, Hypertension, T2D) in either outpatient or inpatient settings at high-performing medical centers.
β’ More than 3 years of cardiology experience specifically focused on heart failure management and guideline-directed medical therapy (GDMT).
β’ Willingness to expand state licensure as Cadence grows its markets and partnerships.
β’ Home office setup with reliable Wi-Fi and robust upload and download speeds; Wi-Fi speed test required during the interview process.
β’ Proven success in fast-paced environments with rapidly changing priorities, workflows, and technology.
β’ Comfortable receiving regular feedback and continuously adjusting processes accordingly.
β’ Highly organized with superior time management skills, capable of balancing patient care with productivity requirements.
β’ Self-motivated and responsible while working remotely with minimal supervision.
β’ Experience in a CHF bridge clinic or T2D clinic, startup environment, and with remote patient monitoring technology is preferred.
β’ Familiarity with AI-assisted clinical tools such as Gemini, Open Evidence, or similar platforms while maintaining clinical judgment.
