This is a fully remote position, open to applicants in Washington.
• Conduct thorough Comprehensive Health Evaluations to evaluate and manage comorbid conditions.
• Attend to patients’ medical, social, emotional, and financial requirements.
• Aim to decrease hospitalizations and enhance patient outcomes.
• Manage patients with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD).
• Work to slow disease progression, cut costs, and prevent readmissions.
• Utilize prescriptive authority, medication management, and diagnostic interpretation.
• Collaborate with nephrologists and multidisciplinary teams.
• Lead the clinical team and ensure that care coordination is in line with medical treatment plans.
• Address the medical and psychosocial needs of patients.
• Follow a full-time schedule from Monday to Friday, with no night, weekend, or on-call duties.
• A strong passion for caring for patients with intricate, chronic conditions.
• Willingness to adapt as the program progresses.
• At least 2 years of experience as a Nurse Practitioner (NP).
• A current, unrestricted NP license in the state of practice.
• DEA license or eligibility within 90 days.
• Current CPR/BLS certification.
• Valid driver's license and insurance in the state.
• Intermediate computer skills, including MS Word, Excel, and Outlook.
• Strong clinical judgment and ability to make autonomous decisions.
• Proficiency in managing complex comorbidities.
• A positive, solution-oriented attitude dedicated to improving outcomes for patients and the team.
• Access to a home office, high-speed internet, and reliable transportation.
• Preferred: 5+ years of experience as a Family NP, Primary Care NP, or in Internal Medicine, Cardiology, Nephrology, or Endocrinology.
• Experience in Value-Based Care or Population Health, including Medicare Advantage documentation and Health Risk Assessments.
• Experience managing population health with a focus on decreasing hospitalizations.
• Previous experience with Cerner or similar Electronic Health Record (EHR) systems.
