πŸ“‹ Description

β€’ Perform comprehensive evaluations of patients' health conditions, requirements, and preferences.

β€’ Create and manage personalized care plans in collaboration with the healthcare team.

β€’ Provide education to patients and their families regarding disease processes, treatment choices, and self-care techniques.

β€’ Advocate for patients' interests within the healthcare framework to secure prompt access to services and treatments.

β€’ Assist with appointment scheduling, referral management, and coordination among various healthcare providers.

β€’ Serve as a bridge between patients with chronic conditions and the healthcare system.

β€’ Report directly to the Program Manager in Clinical Operations.


⛳️ Requirements

β€’ Possession of a Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) license that is active and unrestricted in the state.

β€’ At least 2 years of clinical nursing experience is required.

β€’ Preferred experience in case management, patient advocacy, home health, chronic disease management, or related roles.

β€’ Prior work with Medicare patients and high-risk populations is preferred.

β€’ Strong understanding of healthcare systems, care coordination, and principles of patient advocacy.

β€’ Exceptional communication and interpersonal skills.

β€’ Ability to assess patients' health requirements, formulate care plans, and educate patients on self-management techniques.

β€’ Knowledge of social determinants of health and capability to connect patients with community resources.

β€’ Strong critical thinking and problem-solving skills.

β€’ Proficiency in electronic health records (EHR) and telehealth platforms is preferred.

β€’ Capacity to work independently while engaging in collaboration within a multidisciplinary care team.

β€’ Demonstrated empathy, patience, and dedication to patient-centered care.

β€’ Adaptability in a dynamic and fast-paced healthcare environment.

β€’ Availability between 10 am and 4 pm ET.

β€’ Must reside in the United States.
