Nurse Navigator – Site Disease Group

This is a fully remote position, open to applicants in Florida.

📋 Description

• Act as the primary point of contact for patients entering the healthcare system.

• Manage the scheduling of new patient appointments to ensure timely access.

• Evaluate external medical records, test results, and documentation for initial consultations.

• Identify barriers to care and facilitate referrals to support services.

• Provide education to patients and families regarding diagnoses, treatment options, side effects, wellness, survivorship, and preventive measures.

• Collaborate with multidisciplinary teams, physicians, caregivers, and referring providers to coordinate care.

• Monitor patients throughout the healthcare continuum and identify any bottlenecks or gaps in care.

• Assist with clinical trial eligibility assessments and referrals.

• Conduct psychosocial screenings and refer patients to social workers, financial counselors, and support services.

• Promote shared decision-making and advance care planning.

• Participate in Tumor Board Conferences and coordinate care following the conferences.

• Maintain communication with the tumor registry.

• Record patient encounters and services within the EPIC system.

• Create patient education materials and foster community cancer-support relationships.

• Contribute to the development and enhancement of the nurse navigator program.

• Train new staff and participate in mandatory meetings and training sessions.

⛳️ Requirements

• Graduate from an accredited nursing program.

• Bachelor of Science in Nursing (BSN) is required.

• Current Registered Nurse (RN) license in the State of Florida.

• Basic Life Support (BLS) certification is mandatory.

• At least 2 years of relevant professional experience.

• Compliance with HIPPA, ACHA, JCAHO, and OSHA standards and regulations is essential.

• Completion of mandatory in-services and employee health procedures is required.
