Nurse Navigator

This is a fully remote position, open to applicants in Florida.

📋 Description

• Act as the primary contact for patients entering the healthcare system.

• Manage the scheduling of new patient appointments to ensure timely access.

• Evaluate external medical records, test results, and documentation for initial visits.

• Identify barriers to care and assist in making referrals to support services and community organizations.

• Provide education to patients and families regarding diagnoses, treatment plans, options, side effects, survivorship, wellness, and preventive measures.

• Support patients throughout their care journey, encompassing active treatment, survivorship, chronic cancer management, and end-of-life care.

• Collaborate with multidisciplinary teams, physicians, caregivers, staff, and referring providers to coordinate care.

• Organize appointments, diagnostic tests, procedures, clinical trial referrals, and ensure continuity of care.

• Conduct psychosocial screenings, assist with shared decision-making, and facilitate referrals to social and financial services.

• Participate in Tumor Board conferences and manage post-conference care coordination.

• Monitor patients through EPIC and confirm that required follow-up milestones are achieved.

• Document patient interactions and services rendered.

• Engage in the development of the nurse navigator program, focusing on performance improvement, education, outreach, and staff orientation.

• Adhere to University policies and healthcare regulations.

⛳️ Requirements

• Bachelor's degree in a relevant discipline.

• Active State of Florida Registered Nurse (RN) license.

• At least 2 years of pertinent work experience.

• Capability to foster effective interpersonal relationships.

• Proficient in both oral and written communication.

• Expertise in collecting, organizing, and analyzing data.

• Familiarity with computer software, particularly Microsoft Office.

• Ability to execute job responsibilities independently and as part of a team.

• Strong organizational skills to prioritize tasks and facilitate team progress.

• Knowledge of and compliance with HIPAA, ACHA, JCAHO, and OSHA standards and regulations.
