This is a fully remote position, open to applicants in California.

• Oversee and analyze patient vitals, assessments, and alerts utilizing Welby Health’s platform.

• Offer prompt, evidence-based recommendations and escalate clinically relevant findings.

• Accurately document patient interactions in line with internal protocols and regulatory requirements.

• Create, implement, and modify personalized care plans that address both clinical and social needs.

• Collaborate with physicians, specialists, and community resources to fill care gaps.

• Act as a liaison between patients and healthcare providers to enhance engagement and adherence.

• Provide patient education and coaching through telephone and secure messaging channels.

• Empower patients to effectively manage chronic conditions and develop lasting health habits.

• Identify obstacles to care and address them with practical, patient-focused solutions.

• Represent Welby Health’s healthcare model to patients and families, explaining long-term health management options.

• Bachelor’s degree in nursing or a related healthcare discipline.

• Current and valid registered nurse (RN) license in good standing.

• Approximately two (2) to five (5) years of direct patient care experience in medical-surgical or home care settings.

• Outstanding written and verbal communication abilities.

• Capability to work autonomously in a remote, technology-driven environment.

• Strong organizational skills with a keen attention to detail and follow-through.

• Preferred: active and valid RN license in Illinois or Georgia.

• Preferred: experience in case management, chronic care management, and/or telehealth.

• Preferred: familiarity with remote physiological monitoring, EMR/EHR systems, and patient engagement tools.
