This is a fully remote position, open to applicants in California.
• Coordinate, arrange, and adjust clinical appointments across the care team.
• Oversee referrals to specialists and external clinical services.
• Actively follow up with families to minimize care gaps and reduce no-show rates.
• Facilitate care transitions and ensure continuity within the interdisciplinary team.
• Provide clinical updates to referring physicians.
• Organize lab work and documentation requirements for treatment planning with referring providers.
• Assist providers for patients needing escalation of care.
• Deliver evidence-based education to families regarding the management of chronic conditions.
• Clarify medication regimens, treatment plans, and care objectives in understandable terms.
• Address patient and family inquiries via messaging and phone promptly.
• Promote health literacy and empower families to take an active role in their child's care.
• Initiate and manage medication prior authorization requests with insurance payers.
• Communicate with pharmacies to address dispensing issues and coordinate refills.
• Monitor authorization status and escalate any delays to the relevant clinical team member.
• Maintain precise medication records and document pharmacy communications.
• Record clinical interactions across various EHR and care management systems.
• Engage in interdisciplinary case conferences, team huddles, and quality improvement meetings.
• Report on patient progress and care gaps to clinical supervisors.
• Identify workflow inefficiencies and contribute to solutions for process improvement.
• Active California Registered Nurse (RN) license in good standing.
• Associate's, Bachelor's, or Master's degree in Nursing accepted.
• Proven proficiency with various technology platforms and electronic health record (EHR) systems.
• Excellent organizational, communication, and interpersonal skills.
• Capability to manage competing priorities and collaborate effectively in a remote, virtual environment.
• Comfort in working with diverse, medically and socially complex pediatric populations.
• Enthusiasm for joining a rapidly growing startup and innovating new care models.
• Reliable internet connection and a dedicated workspace suitable for remote work.
• Bilingual in English and Spanish preferred.
• Experience in pediatric, chronic care, or care coordination settings preferred.
• Familiarity with prior authorization processes and payer requirements preferred.
• Experience working in underserved or urban environments preferred.
• At least 1–2 years of experience as a Nurse Care Coordinator.
