Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the daily functions of the social services team, including the implementation of policies, procedures, and collaboration with other clinical providers.

• Attract, onboard, and guide social workers while providing clinical supervision, monitoring practice, and fostering professional development.

• Manage the assessment, planning, and evaluation processes of social services care.

• Guarantee that clinical documentation complies with state, federal, and ACHC regulations.

• Collaborate with the Executive Director on the yearly departmental budget and resource allocation.

• Enhance understanding of community resources and assist patients and referral sources in accessing suitable support.

• Organize in-service training sessions and continuing education opportunities.

• Participate in quality improvement initiatives and explore methods to enhance patient care.

• Undertake additional responsibilities as needed to support patient services.

⛳️ Requirements

• A Master of Social Work (MSW) obtained from an accredited institution.

• A valid California license as a Licensed Clinical Social Worker (LCSW).

• Minimum of 3 years of management experience, ideally in the home care sector.

• Comprehensive knowledge of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to establish trust with patients, families, staff, and the community.

• A proactive, organized, and adaptable mindset—capable of taking initiative and collaborating effectively with others.
