This is a fully remote position, open to applicants in United States.
• Provide leadership from a physician perspective for programs focused on hypertension, diabetes, and CHF.
• Oversee the implementation of evidence-based clinical protocols and care pathways.
• Evaluate and suggest revisions to clinical guidelines in light of new research and advancing standards of care.
• Assist in the escalation of intricate clinical cases.
• Collaborate with Nursing leadership to enhance care delivery workflows.
• Work together with Product and Engineering teams to create AI-enabled clinical workflows, clinical automation, agentic prescription/titration, and decision support systems.
• Assess new product features for clinical safety, usability, and efficacy.
• Aid in defining future clinical capabilities that enhance patient engagement and outcomes.
• Track program performance and clinical quality metrics.
• Identify opportunities to enhance patient outcomes, engagement, and operational efficiency.
• Promote quality improvement initiatives and conduct clinical performance reviews.
• Assist in developing reports for payer clients, detailing clinical outcomes and utilization trends.
• Ensure that clinical programs are in accordance with evidence-based medicine and regulatory standards.
• Participate in clinical governance committees and quality review processes.
• Support accreditation, compliance, and audit activities.
• Collaborate with Legal and Compliance on clinical policies and documentation.
• Provide education to internal teams regarding clinical best practices and emerging evidence.
• MD with an active, unrestricted license to practice medicine in the United States.
• Board Certification in Internal Medicine, Cardiology, Endocrinology, or a related specialty.
• A minimum of 5 years of clinical practice following residency.
• Proven clinical expertise in managing hypertension, diabetes, congestive heart failure (CHF), and other chronic conditions prevalent in the Medicare population.
• Experience in developing, implementing, or overseeing evidence-based clinical protocols and care pathways.
• Background working with interdisciplinary care teams, including physicians, nurse practitioners, nurses, and care coordinators.
• Preferably experienced with digital health, virtual care, Remote Patient Monitoring (RPM), or Chronic Care Management (CCM).
• Knowledgeable about value-based care models, CMS quality programs, HEDIS, and Star Ratings.
• Experience collaborating with health plans or provider organizations.
• Familiarity with AI-enabled clinical decision support tools and healthcare technology.
