Medical Assistant, Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee patient census with a focus on resolving issues to bridge gaps in both clinical and non-clinical patient care.

• Identify and connect patients with community resources that can enhance their care.

• Educate patients and promote health literacy regarding the management of chronic conditions.

• Conduct medication management, which includes identifying potential medication issues, performing reconciliations, ensuring adherence, and coordinating refills.

• Help guarantee the timely provision of Home Health, DME, Home Infusion, and other essential services.

• Address patients’ inquiries and foster open communication to gain insight into their needs.

• Assist with or oversee referrals and appointment scheduling.

• Collaborate effectively with providers to implement value-based care management initiatives.

⛳️ Requirements

• Certified Medical Assistant with an active CCMA certification.

• Minimum of 3 years of hands-on experience in direct patient care.

• Proven experience in identifying resources and coordinating requirements for patients managing chronic conditions.

• Active CCMA certification is mandatory.

• Proficient in Microsoft Office Suite.

• Exceptional written and verbal communication abilities.

• Reliable high-speed Wi-Fi connection.

• A home office or workspace compliant with HIPAA regulations.

• Capacity to sit for extended periods.

• Ability to articulate clearly and listen attentively, often over the phone, for prolonged durations.

• Competence in understanding, processing, and taking detailed notes in real-time during phone conversations.

• Experience with Epic is advantageous.

• Bilingual proficiency is a plus.
