This is a fully remote position, open to applicants in United States.
• Lead a multi-specialty polychronic care model in-home within the designated Georgia market.
• Contribute to the development of clinical strategies, policies, programs, processes, protocols, guidelines, and operational procedures.
• Supervise daily clinical and business operations through direct patient care, care management, social worker assistance, and pharmacy services.
• Review and authorize care plans for APP, RN, social workers, and pharmacy personnel.
• Review, approve, and co-sign encounters conducted by APPs.
• Ensure compliance with evidence-based clinical pathways and established clinical guidelines.
• Document clinical activities, interventions, and tasks within Salesforce and Athena.
• Maintain accountability for the total cost of care, Medical Loss Ratio, clinical interventions, utilization, prescribing practices, therapy management, and HEDIS/Gap Closure metrics.
• Conduct daily high-risk and concurrent review rounds.
• Directly oversee frontline clinical and operational team members, including regular field shadowing and ride-alongs.
• Delegate operational tasks to the Market Manager, including patient engagement, scheduling, administrative management, strategic implementations, and P&L oversight.
• Foster and uphold a market culture centered on clinical excellence and ownership of patient outcomes.
• Evaluate and present market performance and outcomes to Executive and Senior Leaders.
• Provide both direct and indirect patient care, encompassing the diagnosis and treatment of acute and chronic conditions.
• Engage patients in their treatment plans and evidence-based care pathways.
• Collaborate with community physicians, facilities, health plans, and partners in peer-to-peer and direct patient care decision-making processes.
• Conduct peer-to-peer consultations.
• Order laboratory tests and referrals, close care gaps, and complete clinical interventions.
• Guide population health management efforts for high-risk patients, focusing on reducing admissions/readmissions, post-discharge visits, and enhancing visit quality.
• Collaborate with Medical Economics, Finance, and stakeholders to analyze utilization trends.
• Present population health strategies during clinical and operational meetings.
• Conduct patient reviews aimed at high utilizers, high-risk patients, high-opportunity patients, and specific patient cohorts.
• Participate in the Monogram on-call rotation, committing to a minimum of 7 days on call each quarter.
• Provide coverage for other Market Physician Executives during their PTO or vacancies.
• Active state medical license without restrictions and free of sanctions from Medicaid or Medicare.
• Willingness to obtain licensure in multiple states.
• MD (Medical Doctor) or DO degree from an accredited medical institution.
• Board Certified (BC) or Board Eligible (BE) in an ACGME-approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
• Active, unrestricted state medical license required in each state within the market, with the ability to secure additional state licenses as necessary.
• Proven experience in applying evidence-based clinical criteria.
• Experience in multispecialty, geriatrics, and/or value-based care environments.
• Advanced management and communication abilities.
• Experience working with high-need Medicare Advantage and managed Medicaid populations.
• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment.
• Remote work within the designated market with occasional in-home patient treatment visits and some domestic travel required.
