This is a fully remote position, open to applicants in Mississippi.
• Oversee performance in relation to clinical and operational benchmarks.
• Inspire, guide, and ensure accountability within a team of nurse practitioners, registered nurses, and social workers.
• Prepare, evaluate, and interpret performance and health outcome reports to pinpoint areas that need focus and enhancement.
• Act swiftly to address known performance and health outcome challenges.
• Organize staffing schedules to maximize efficiency and productivity.
• Assist in the recruitment of clinical staff members.
• Implement corporate and regional strategies to ensure high-quality patient care.
• Collaborate closely with clinical and corporate leaders to guarantee outstanding patient care.
• Drive market performance in areas including admissions, blood pressure and diabetes management, conservative pathways execution, dialysis initiations, and staff productivity.
• Undertake additional responsibilities as assigned.
• Uphold the mission and values of Monogram Health.
• Must reside in the state of Mississippi.
• Basic Life Support (BLS) certification is essential; the company facilitates certification completion during onboarding.
• Experience in supervising nurses, nurse practitioners, and social workers.
• Demonstrated accountability for performance and achieving results.
• Capacity to multitask and manage numerous concurrent priorities.
• Exceptional verbal, written, and visual communication skills.
• BS/BA degree or equivalent experience.
• A minimum of 3 years of experience in Managed Care or Delegated Risk Partnerships.
• Preferred experience in managing a high-risk patient population and leading clinical staff in care management, managed care, and/or home health settings.
• Familiarity with Microsoft Excel is preferred.
• Experience with Salesforce is preferred.
• Experience in analyzing clinical and operational performance.
• The position may require 25% travel within the designated market.
