This is a fully remote position, open to applicants in Arizona, +10 more states.
• Oversee and manage Enhanced Care Management staff to address the medical, behavioral health, and social needs of clients.
• Engage in communication with ECM staff and, at times, patients via telephone, telehealth, or during Interdisciplinary Care Team meetings.
• Discuss medication management and adherence while ensuring proper follow-up with primary care, behavioral health, and specialty providers.
• Provide guidance during crises faced by ECM staff.
• Collaborate with clients’ comprehensive care teams to advocate for and ensure access to necessary services.
• Assess progress towards individualized care plan objectives.
• Monitor team performance, coach staff, and manage overall performance outcomes.
• Supervise approximately 10–15 direct reports.
• Oversee client outreach, enrollment processes, and the development of Individual Care Plans.
• Act as an escalation point for addressing complex client needs.
• Propel team performance in alignment with Key Performance Indicators (KPIs).
• Partner with Enhanced Care Management Directors on process enhancements and changes.
• Provide clinical oversight roughly 30% of the time, which includes staff supervision, clinical sign-offs, care plan oversight, IDT meetings, and clinical consultations.
• Deliver direct care management as necessary.
• Travel up to 20–25% for training, meetings, and other business-related activities.
• Must possess an active, unrestricted RN license and the ability to secure California licensure within 6 months of employment.
• Willingness to obtain additional state licensure as required.
• Minimum of 2 years of experience in people management (4 years preferred).
• 2–3 years of experience in care management.
• 1–2 years of post-acute care experience.
• Experience with change management in fast-paced growth settings.
• Strong organizational skills and ability to work independently.
• Proficient in Microsoft and Google applications.
• Experience with Salesforce is advantageous.
• Excellent communication and motivational skills, including conflict resolution abilities.
• Familiarity with Lean Six Sigma methodology is preferred.
• Availability to work Monday–Friday, from 8:30 am to 5:00 pm Pacific Time, with potential for evenings and weekends.
• Must be legally authorized to work in the United States.
• Completion of required background screening is mandatory.
