This is a fully remote position, open to applicants in Arizona, +10 more states.
• Provide clinical oversight and management for Enhanced Care Management staff to address the medical, behavioral health, and social needs of clients.
• Engage with ECM staff and occasionally with patients through telephone, telehealth, and Interdisciplinary Care Team meetings.
• Discuss medication management and adherence, ensuring proper follow-up with primary care, behavioral health, and specialist providers.
• Guide ECM staff in managing crises.
• Collaborate with the broader care teams of clients to advocate for their needs and ensure the provision of necessary services.
• Evaluate progress towards goals in individualized, patient-centered care plans.
• Monitor team performance, provide coaching to staff, and manage performance as necessary.
• Supervise approximately 10–15 direct reports.
• Oversee client outreach, enrollments, and the development of Individual Care Plans.
• Assess whether client needs can be managed by an ECM Lead Care Manager and serve as an escalation point.
• Drive the team to achieve Key Performance Indicators (KPIs).
• Work collaboratively with Enhanced Care Management Directors to implement changes and process improvements.
• Provide clinical oversight about 30% of the time, which includes supervising clinical staff, clinical sign-offs, care-plan oversight, IDT meetings, and consultation on complex cases.
• Provide direct care management services as necessary.
• Travel up to 20–25% for training, meetings, and other business-related needs.
• Active, unrestricted LCSW or LMFT license in California is required.
• Willingness to obtain additional state licensure as required.
• Minimum of 2 years of experience in people management (4 years preferred).
• 2–3 years of experience in care management.
• 1–2 years of experience in post-acute care settings.
• Availability to work Monday through Friday, from 8:30 AM to 5:00 PM Pacific Time, with flexibility for potential evenings and weekends.
• Demonstrated leadership experience focused on results, particularly in change management within rapidly growing environments.
• Strong organizational skills and the ability to work independently.
• Proficiency in Microsoft and Google applications.
• Experience with Salesforce is a plus.
• Excellent communication and motivational skills, including conflict resolution strategies.
• Preferred knowledge of Lean Six Sigma methodology.
• Must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, Utah, or Washington.
• Legally authorized to work in the United States.
• Background screening must be completed prior to employment.
• Employees in patient-, client-, or customer-facing roles must be vaccinated against influenza, subject to accommodation review.
