This is a fully remote position, open to applicants in Kansas.

📋 Description

• Create, evaluate, and manage comprehensive care management activities to achieve quality, cost-effective healthcare results.

• Develop or assist in crafting personalized service care plans for long-term care members.

• Educate members and their families/caregivers about available services, benefits, procedures, healthcare instructions, referrals, and service alternatives.

• Assess member needs and available resources, recommending or facilitating suitable plans.

• Identify providers, specialists, and community resources for long-term care.

• Serve as a liaison between members, families, caregivers, and care provider teams to ensure accessible services.

• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care requirements.

• Monitor care plans, member status, and outcomes, suggesting updates based on member needs.

• Collaborate with long-term care healthcare providers and partners.

• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.

• Conduct home and/or site visits to evaluate member needs and work in partnership with providers and stakeholders.

• Provide leadership with feedback on opportunities to enhance care quality and cost-effective service delivery.

• Execute additional tasks as assigned while adhering to policies and standards.


⛳️ Requirements

• Must live in Lyon, Osage, and/or Coffey County, Kansas.

• Bachelor’s degree with 2–4 years of relevant experience; or a bachelor’s degree in health, human services, social work, or education with at least one year of qualifying experience.

• Alternatively, a high school diploma or equivalent plus three years of relevant experience in case management for elderly individuals, including those with behavioral health issues, physical or developmental disabilities, or HIV/AIDS.

• Equivalent experience may be accepted based on applicable knowledge, responsibilities, scope, and skills.

• Field-based work required.

• Technologically proficient.

• Exceptional communication and customer service abilities.

• Familiarity with HCBS waiver requirements.

• Strong time management skills.

• Ability to work collaboratively in a team setting.

• Knowledge of community-based resources and referral processes.

• Effective problem-solving abilities.

• Skills in assessment and care planning.

• Adaptability and flexibility.

• Capacity to comply with current state, federal, and third-party payer regulations, policies, and standards.
